Hola Yisus proba este formulario, es muy basico y sencillo.
Los datos te llegan a travez de un mail que manda el interesado, pero desde su cuanta de mail POP3. Fijate si te sirve, igualmente tenes que hacer algunos cambios, como por ejemplo el mail a donde queres que te llegen los datosy otras pabadas.
